Pfizer-BIONTech COVID-19 Vaccine Clinic at WACS 

Friday, May 21, 2021 (Second dose on Friday, June 11, 2021)

Beginning at 1:30 p.m. in the half gym – WACS Secondary School

Eligible Individuals: WACS students (both in-person and remote) age 12 and older & their parents/guardians; WACS employees

No children will be vaccinated without explicit, written consent from a parent or guardian. Any parents/guardians who would like to have their 12 to 17-year-old child(ren) vaccinated must complete a consent form for each child (linked below) and send it with your child(ren) to school on or before Friday. Consent forms should be turned into the nurse's office.

The Chautauqua County Department of Health will be offering a free and voluntary Pfizer-BioNTech COVID-19 vaccine clinic to all remote and in-person students ages 12 and older and their parents/guardians at Westfield Academy and Central School on Friday, May 21st.

The clinic will open at 1:30 p.m. in the secondary school half gym.  If you would like one or more of your children to receive the vaccine, you must first complete an online registration form. 

The clinic will return on June 11th to administer the second vaccine dose.  Please contact our nurse's office if you experience difficulty registering online.  We will assist you with the process.  

Under current State guidance, individuals who are vaccinated and exposed to COVID - 19 are not required to quarantine.   We appreciate your continued support and partnership.  We look forward to a positive remainder of the school year.
